For the past 14 years, American women have been restricted in their choices due to the 1992 “ban” on silicone gel breast implants imposed by the FDA in the United States. The FDA’s recent approval of silicone gel breast implants was an important step towards providing American women state-of-the-art options to enhance their breast shape, form, and proportions. According to Plastic Surgeon Dr. G. Patrick Maxwell, in Nashville, Tennessee, and one of the inventors of breast implant technology, “Saline implants, having a less natural feel and shape, essentially relegated breast augmentation to a volume enhancement operation until recently. These devices will enlarge breasts but offer little option for the aesthetic considerations of form and proportion which truly define female beauty. Although it has taken many years to prove the safety of silicone gel, silicone breast implants are now the single most studied medical device in the history of medicine.”

The cohesive silicone gel in these implants is like a thick hair gel, which is less liquefied than previous models where leakage sometimes occurred. Still under evaluation by the FDA are the next generation of form-stable, highly cohesive silicone gel-filled implants, with a consistency similar to a “gummy bear”. “These implants have shown improved outcomes in aesthetics, reduced capsular contracture and longevity and will dictate future development and improvements within the industry,” says Dr. Maxwell, whose new book, Breast Beauty (MDPUBLISH.COM) will be published in early 2008.

Most women have a preconceived notion of what size and shape breast they desire before they ever set foot in a plastic surgeon’s office. However, their aesthetic goals have to be weighed against the quality and quantity of breast tissue and measurements of each woman. Modern implant selection, preoperative planning and surgical techniques are far more sophisticated today, and the best approach to breast enlargement, enhancement or restoration is indeed very individual. Contrary to popular belief, breast augmentation is not the same for every woman and involves much more than just placing an implant. Each surgeon evaluates a patient on the basis of precise measurements and calculations to select the most appropriate width or base of the implant, the height of the implant and the preferred implant projection. The site, or pocket, into which the implants are placed and incision options should also individualized to provide a fully custom breast procedure. In summary, there have been great strides made in the field of breast reconstruction in the last five years, and women who have undergone skin sparing breast cancer surgery are afforded a wider array of good choices now. In terms of breast enhancement, uplifts and augmentations, bigger is not necessarily better and the natural look is more desirable in many regions across the U.S.
